This consultation starts with what older people have already told us they need from health and social care. It focuses on outcomes and how we can achieve these by working together in partnership. The strategy will provide the framework on which health and social care services for people aged 65 and over will be commissioned over the next 10 years, together with the commitment of NHS Grampian and Aberdeen City Community Health and Social Care Partnership to work in partnership with our citizens and other organisations, including our Change Fund Partners (ACVO and Scottish Care) to provide high quality, responsive services that deliver on the desired outcomes for the older people of the City.
